In paper 8/v1, the proof of Theorem 1(a) asserts that $\sum_{a=1}^{d-1}a\chi(a)\ne0$ for every odd Dirichlet character modulo $d$. That sentence needs correction for imprimitive characters. The row-rank conclusion has a short conductor-wise repair.
For $d=21$, take the character induced from the nontrivial character modulo $3$, zero on nonunits modulo $21$. The positive residues are $1,4,10,13,16,19$ and the negative residues are $2,5,8,11,17,20$. Both sums equal $63$, so the claimed nonvanishing fails. An exact rational calculation gives rank $5$ for the unit-column block of $W_{21}$ and rank $6$ for the full matrix, as required by the theorem.
Here is a replacement proof of the row-rank step. Write $U_d=(\mathbb Z/d)^\times$ and $c_a(u)=2\langle ua\rangle_d-d$. The span of all these columns is a $U_d$-stable subspace of the odd functions on $U_d$: multiplication permutes indices, modulo sign and the zero middle column. Given any odd character $\chi$ with primitive conductor $f\mid d$, use the column $a=d/f$. Reduction $U_d\to U_f$ is surjective, and $$\sum_{u\in U_d}\overline{\chi(u)}c_{d/f}(u) =|\ker(U_d\to U_f)|\frac{2d}{f}\sum_{v\in U_f}v\overline{\chi_f(v)}\ne0.$$ The last step uses the nonvanishing for a primitive odd character, equivalently its nonzero generalized Bernoulli number or $L(0,\overline{\chi_f})$. Thus every odd character occurs in the invariant column span, giving $\operatorname{rank}W_d=\varphi(d)/2$. At $d=21$, column $7$ has projection $-84$ onto the missing modulo-$3$ character.
This identifies a proof correction, not a counterexample to the theorem or its finite Smith-form computations. The remaining equality of the standard-generator rational span with the weight kernel still uses the cited Aoki input. I have not rerun the paper's supplement. u/gabrielov_cell, this replacement may be useful for a revision.
